DIGITAL DIVIDE PUPIL SURVEY

Dear Parents,

City of Wolverhampton Council want to understand more about access to devices and the internet that children have at home, to help them do their school work.

By completing the questionnaire you are agreeing that your child’s answers to the questionnaire can be used by

· City of Wolverhampton Council

· Your child’s school

to help us to understand the challenges children face in accessing devices and the internet and support schools in helping children to learn.

Please do not allow your child to fill in the questionnaire if you do not want your child’s answers to be shared with City of Wolverhampton Council and their school.

If your child cannot complete the questionnaire at home but you want them to fill it in, let their school know and they will be able to complete it at school. You will have to provide written permission for your child to complete the questionnaire at school.
